Who We Accept
Clear Sky Adult Family Home serves elderly and vulnerable adults who need residential care and supervision in a home-like setting. We are well-suited for residents with the following needs:
- Age
- Adults 18 and older (primarily serving seniors 60+)
- Care Level
- Personal care and supervision — not acute medical care
- Diagnoses
- Dementia, Alzheimer's, mobility limitations, chronic illness, general aging needs
- Payers Accepted
- Medicaid (ALTSA/DDA), Private Pay
- Availability

Payer Information
We accept the following forms of payment for residential care services.
Medicaid (ALTSA / DDA)
Accepted- Certified to accept WA State Medicaid through DSHS ALTSA and DDA programs
- Case managers can submit referrals directly — we work with your office throughout
- All required DSHS documentation and care planning handled on our end
- Contact us for current Medicaid availability
Private Pay
Accepted- Families who are self-paying may contact us directly to inquire about rates and availability
- We provide a written service agreement outlining care services and fees
- Long-term care insurance may be applicable — documentation provided as needed
- Flexible intake process for private pay residents
How to Refer a Client
For DSHS case managers, hospital discharge planners, and social workers:

Share the Client Profile
Provide a summary of the client's care needs, diagnoses, current medications, payer source, and any behavioral considerations. This helps us determine fit.
Schedule a Pre-Admission Assessment
We conduct a pre-admission assessment to ensure we can meet the client's needs — in person, by phone, or through documentation review.
Complete Required Documentation
We will work with you to complete all DSHS-required paperwork, including the care plan, service agreement, and financial eligibility documentation.
Move-In and Transition
We coordinate with you, the family, and any transitioning providers to ensure a smooth move-in. We are experienced in hospital-to-AFH transitions.

If you are searching for a placement for a family member, you are welcome to contact us directly. We understand this is one of the most important decisions a family will make.
We encourage families to call us, ask questions, and schedule a visit. We will take the time to explain what daily life looks like at Clear Sky, introduce you to our team, and honestly discuss whether we are the right fit for your loved one's needs.
If your family member is currently on Medicaid, your DSHS case manager can initiate the referral process. If you are private pay or transitioning from a hospital, you can contact us directly.
